Date of Assessment: 9 June to 28 July 2025. The Horizon (New) Ltd is a care at home service providing support to adults of all ages living with a range of different needs including people who require end of life care. At the time of assessment, the service was supporting 1 person who was receiving personal care. The provider is also registered to provide supported living services to people, but this element of their service provision was not assessed during this assessment as there was no one in the supported living service receiving personal care. CQC only assesses and inspects where people receive personal care. This was the first inspection of this service since CQC registration on 3 May 2022. We looked at all 5 key questions. The overall rating for this service was good. Staff managed people’s risks to care well. The service had systems to support the safe administration of medication to people. Staff knew people well and understood their needs. They supported people to live healthier lives and where possible, reduce their future needs for care and support. We found staff had received mandatory training and were confident in describing how they would put training linked to safeguarding, whistleblowing and the Mental Capacity Act into practice. The provider held records of consent from people however, documentation of consent for medication needed improvement as they were inconsistently recorded. Staff assessed people’s need and people had care plans to ensure care was delivered in a person-centred way. The provider worked closely with health and care professionals to deliver people’s care as per their expectations. The provider used systems and had processes to support the recruitment of staff who had been assessed as safe to work with vulnerable adults. However, more information on employment history was required on application forms to make them complete. Members of staff were kind and caring. They delivered care with compassion and dignity. Staff recorded people’s likes, dislikes and preferences in their care records. The provider’s vision and values focused on providing care that was individually tailored to people and to provide a high quality of care. Staff told us they enjoyed working for the service and felt well supported. We received positive feedback about the registered manager.
