The assessment took place from 12 to 25 February 2026. Mira Social Care is a domiciliary care service that has been registered with the CQC since February 2023. This was the provider’s first inspection since registration. At the time of assessment, the service was supporting two people. It provides care at home to older people, younger adults, and adults of all ages living with dementia, mental health conditions and physical disabilities. We found that some areas of the service required improvement because care was not always delivered safely and governance systems did not provide effective oversight of quality or risk. Records were inconsistent, outdated or incomplete, and quality assurance systems had not identified gaps in medicines management, risk assessments or environmental safety. These shortfalls reduced assurance that people consistently received safe care. People and relatives described staff as kind, respectful and reliable. Staff knew people well, provided continuity, and supported them in ways that maintained dignity, comfort and familiarity. The registered manager was visible, and staff felt supported, describing good communication and approachable leadership. These positive experiences contributed to people receiving good care. However, documentation did not always reflect people’s current needs. Care plans, risk assessments and medicines records contained omissions or contradictory information, including missing details about falls risks, dementia‑related needs, catheter care and environmental hazards. Medicines documentation included gaps in medication administration records (MAR) charts, inaccuracies and missing topical medication records, meaning staff relied heavily on verbal updates rather than accurate written guidance. Governance arrangements were not effective. Policies were not consistently reviewed, audits were not routinely completed, systems to review and maintain employment records were not robust, and gaps had not been identified or addressed. Leaders therefore did not always act on the most accurate information about risk or quality. We found breaches of legal requirements relating to regulation 12 (safe care and treatment) and regulation 17 (good governance).
