Date of Inspection: 2 July to 17 July 2025. This inspection was carried out in response to concerns we received about the service in relation to wound management and learning lessons. Desire Care -Burton is a domiciliary care agency supporting older people, younger adults, people with physical disabilities, sensory impairment, people with learning disabilities or autistic people or people who are living with dementia. We expect health and social care providers to guarantee people with a learning disability and autistic people, respect, equality, dignity, choices and independence and good access to local communities that most people take for granted. ‘Right support, right care, right culture’ is the guidance CQC follows to make assessments and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it. The provider did not always have a proactive and positive culture of safety based on openness and honesty. They did not always work well with people and healthcare partners on improving people’s lives or protecting them from avoidable harm and neglect. The provider did not always make sure there were enough qualified, skilled and experienced staff and not all staff records showed they received regular supervision and development. They did not always make sure the administration of medicines and treatments were effectively recorded. Whilst the provider worked with people and healthcare partners to meet people’s needs, further information was required to ensure safety was managed or monitored. The provider did not always make sure people’s care was effective, as people’s records were not always up to date and reflective of their current needs. Decisions around people’s consent when delivering care and treatment was not always clearly recorded. Staff shared a culture of working together to meet people’s needs, although a shared strategy was required to ensure improvements were identified and actioned across the organisation. Leaders did not all understand the context in which care, treatment and support was required to be delivered and the provider did not have clear responsibilities, roles, systems of accountability and good governance. The provider did not always understand their duty to collaborate and work in partnership at a high level, and they did not always focus on continuous learning, innovation and improvement across the organisation and local system. The provider did however, foster a positive culture where people felt they could speak up and their voice would be heard and the provider valued diversity in their workforce. We have found a breach of regulation at the service relating to good governance. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded.
