Date of Assessment: 27 May to 23 June 2025. Pinnacle Caring Services Ltd is a Domiciliary Care Agency (DCA) and provides personal care to people living in their own homes. At the time of the assessment the service supported 2 people with live-in care. The service is registered to support older people, people with physical disabilities, mental health conditions and learning disabilities. However, the service did not currently support anyone with a learning disability and the provider had no immediate plans to do so. We considered the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed people with a learning disability and autistic people respect, equality, dignity, choices, independence and good access to local communities that most people take for granted. This was a planned assessment of all quality statements under the key questions of safe, effective, caring, responsive and well-led. Since our previous inspection, the provider had implemented some quality monitoring systems. However, these needed to be further strengthened to ensure quality improvements were always identified and actioned. Whilst overall people received their medicines as prescribed, further work was required to ensure record keeping supported the safe administration of medicines. People were supported by staff who were consistent and familiar Whilst staff understood people’s needs and promoted person-centred care, risk assessments and care plans were not always fully up to date and reflective of people’s needs. Some records needed to be more detailed to demonstrate how people had been involved in reviews and decisions about their care. The provider was responsive to our feedback and took some immediate action, however, they needed to proactively identify where improvements were required in future. The provider worked in partnership with others to ensure people’s needs were assessed and effectively met. Staff undertook relevant training and understood how to safeguard people. People were supported to give feedback and the provider listened and addressed any concerns. The provider promoted a culture of openness and learning. However, they needed to ensure regulatory requirements in relation to notifying CQC of changes within certain staff roles were met. They addressed this straight away following our feedback. At our last inspection the service was in breach of regulations regarding governance. The service had made some improvements but remained in breach of this regulation. We have asked the provider for an action plan in response to the concerns found at this assessment.
