Our assessment started on 9 August 2025 and finished on 26 September 2025. We carried out our on-site comprehensive assessment on 13 August 2025 to follow up on the warning notices issued at the last assessment on 16 December 2024 and review the improvements made. Kemfa Services Limited provides personal care and support to people who require assistance in their own home. At the time of our assessment 47 people were being supported by the service. The service is registered to support people of all ages including those with dementia, people with a learning disability and autistic people, people with an eating disorder and physically disabled people. They are also registered to provide services for children of all ages. However, at the time of the assessment, they were not providing services to children or people with eating disorders. An assessment has been undertaken of a specialist service that is registered for use by autistic people or people with a learning disability. At the time of the assessment, the service was not used by anyone with a learning disability or an autistic person. However, we assessed the care provision under our guidance 'Right support, right care, right culture', as it is registered as a specialist service for this population group. At our last assessment on 16 December 2024, we found the provider was in breach of the regulations in relation to safe care and treatment and good governance. At this assessment the provider had made significant improvements in all areas and was no longer in breach of the regulations. The provider now had systems in place to monitor the service, which provided good oversight to monitor and improve outcomes for people. People were safely supported and risks to their health and wellbeing were well managed. The provider understood their responsibilities to keep people safe from harm and abuse. Medicines were managed safely and staff followed infection control policies and procedures. There were enough staff to meet people’s needs, and they had been trained and supported to understand their role. The provider took appropriate actions and had systems in place for learning when things went wrong and made changes to improve the service. People were involved in the planning and delivery of their care. Staff sought people’s consent before providing care. Where people could not consent to care, a representative was appointed as the agreed decision maker for more complex affairs. People had their needs assessed before care commenced and how they wished their care to be delivered was recorded. People were supported to eat and drink enough. There were effective approaches to monitor people’s care and treatment and their outcomes. Staff respected people’s privacy and dignity and encouraged people to remain independent. People were supported by a caring staff group who respected their choices and preferences. Staff felt supported by the registered manager and colleagues and felt able to contribute to decision making. People were supported to make decisions about their own care and support. Staff treated people as individuals, providing a person-centred approach. People had access to health care professionals to ensure their health needs were met when required. People’s future planning and end of life arrangements were documented where discussed to record their wishes. The team had improved systems to manage and monitor the safety and quality of the service. The provider had learned lessons, strengthened management arrangements, and developed an ongoing plan for continued improvement .

We looked at all 5 key questions, but we did not assess all quality statements during this assessment. For those areas we did not assess, we used the ratings awarded at the last inspection to calculate the overall rating. At the last assessment the service was rated as good. At this assessment we found the rating had deteriorated and the provider was in breach of legislation for safe care and treatment and good governance. The provider's processes for monitoring the safety and quality of the service were not effective and had failed to identify the significant concerns found during the inspection.
