Key weaknesses identified were inconsistent communication with families and continued reliance on agency staff, while medicines management, safeguarding awareness, and person-centred practice were notable strengths.
Realise South West received a Good rating across all five key questions at its first comprehensive inspection since relocating in 2019, demonstrating safe, person-centred care for 43 people with learning disabilities, autism and mental health needs. Minor shortfalls were identified in supervision consistency, care plan review frequency and documentation of consent restrictions, all of which the registered manager had recognised and was actively addressing.
